Article 1: Information Required for Inspection

1. Inspection Report Form for Motor Vehicles in the Factory (two copies)

2. Forklift certificate (one original and one copy each)

3, a copy of the ownership unit code certificate.

4, a copy of the business license of the owner.

5, not directly handled by legal persons (need to submit a letter of entrustment)

Article 2: Information Required for Application for Driving Permit

1. Special equipment registration form (two copies)

2. Inspection certificate (one original and one copy respectively)

3. Forklift certificate (one original and one copy each)

4, a copy of the ownership unit code certificate.

5, a copy of the business license of the owner.

6, not directly handled by legal persons (need to submit a letter of entrustment)

7. One copy of forklift management system and catalogue

8. One original and one photocopy of the driver's license (note that the employing unit shall affix its official seal)
